Release of Information Form

Please print the ROI form, complete and return it to Lincoln Trail Behavioral Health System via fax – 270-351-8075, via mail – 3909 South Wilson Road, Radcliff, Kentucky 40160 or you may bring it to our facility.  There may be a 30 day waiting period to process this request. 

Proof of Guardianship
Upon admission to Lincoln Trail Behavioral Health System, proof of legal guardianship will be required especially if there is more than one guardian or joint custody.  Please bring any guardianship papers you may have to the facility. The Intake Department will have forms to be completed upon admission.
