Adult Chemical Dependency Program The Adult Chemical Dependency Inpatient Program at Lincoln Trail Behavioral Health System is highly structured for adults over the age of 18 with a need for supervised detoxification or for a recovery program. The philosophy of the Adult Chemical Dependency Program centers on the Recovery Dynamics Model. The Recovery Dynamics Model enables counselors to have a broader and deeper understanding of the Alcoholics Anonymous plan of recovery and to be able to present the program to individuals clearly and effectively. This model is a clear and concise explanation of the 12 Step Program and is a proven program that is readily available to individuals for continuing support after they leave Lincoln Trail Behavioral Health System. The multidisciplinary treatment team is led by an Addictionologist or Psychiatrist and includes a Certified Alcohol and Drug Counselor, Case Coordinator, Social Worker, Nurse and Activity Therapist. The treatment is individualized and guided by a tailored treatment plan that addresses specific needs. The Adult Chemical Dependency Program components include: Medical Detox Chemical Dependency Education/Recovery Dynamics Individual Therapy Group Therapy Family Therapy Activity Therapy Spirituality Group Nursing Education Relapse Prevention AA/NA Meetings Lincoln Trail Behavioral Health System's Chemical Dependency Program is also designed to treat individuals with co-occurring disorders. Individuals are able to attend group sessions addressing their psychiatric and chemical dependency needs. These needs will also be addressed in individual therapy as well as with the attending physician. Discharge Planning begins upon admission by the individual's Case Coordinator to develop goals and plans for a transition to a less intensive level of treatment. Adult Psychiatric Program The Adult Acute Inpatient Psychiatric Program at Lincoln Trail Behavioral Health System provides treatment for men and women, ages 18 and over, who suffer from a wide range of serious psychiatric illnesses including: Suicidal Feelings/Thoughts Depression Anxiety/Panic Attacks Schizophrenia Phobias PTSD (Post Traumatic Stress Disorder) Bi-Polar/Manic Depressive Disorder Extreme Anger and/or Dangerous Behavior Alcohol/Drug Dependency with Psychiatric Issues Lincoln Trail Behavioral Health System's Adult Program offers a structured, safe and caring environment that teaches positive self-awareness, new coping skills and provides opportunities for personal development. The program allows adults and their families to work through a crisis, quickly stabilize and return to the appropriate level of care after discharge. The Adult Psychiatric Inpatient Program is designed to enable the individual to function more effectively in family, work and social environments. Lincoln Trail Behavioral Health System recognizes the importance of family involvement and support in the treatment process, therefore the program emphasizes family participation through therapy. A psychiatrist-led multidisciplinary team of mental health professionals (Nurses, Case Coordinators, Social Workers, Mental Health Technicians, Activity Therapists and Substance Abuse Counselors) directs the treatment process. Individuals participate in different elements of the program based on his/her individual needs. These elements include: Individual Therapy Family Therapy Group Therapy Activity Therapy Psychiatric Education Nursing Education Chemical Dependency Education Relapse Prevention The individual's Case Coordinator begins discharge planning upon admission by developing goals and plans for a transition to a less intensive level of treatment. The Willows Program The Willows Program at Lincoln Trail Behavioral Health System is one of the few such programs in the United States treating sexually reactive adolescent females from the ages of 9 - 17. The focus of the program is assessment, diagnosis and the completion of treatment to address sexually reactive behaviors, cycle of offense and relapse prevention. Some of the treatment goals for this specialized program are: Accept responsibility for sexually offending behaviors Commit to changing behaviors and thought processes that lead to offending behaviors Identify thinking errors and cognitive distortions that contribute to the offending acts Learn how to meet emotional and sexual needs without harming others Develop victim empathy Learn the sexual abuse cycle and identify the triggers to offending The Willows Program includes Juvenile Sex Offender Groups, Individual, Group and Family Therapy, Nursing Education, Psychiatric Education and Activity Therapy. Each individual attends school on Lincoln Trail's campus at Heartland Academy. Staffed by Kentucky Certified Teachers, the school offers a minimum of six hours of individual instruction and study daily. Admission Criteria: History of committing sexual offense or sexually reactive behaviors. Must be between the ages of 9 - 17. Full Scale IQ of 70 or above. May have been a sexual abuse victim. Potential for harm to self or others. Possible history of self-abusive acts. The Willows Program has a Clinical Review Team that works closely with the Assessment and Referral Department in determining if referrals meet appropriate criteria. Length of Program: The length of stay is dependent on the level of commitment to treatment from the individual. The average length of stay is approximately 9 - 12 months. 1.What items can or should I bring to the hospital with me? Items that should be brought to the hospital: Undergarments Socks Shirts Pants/Shorts Pajamas Tennis Shoes (1 pair) Long sleeve shirt/sweatshirt Coat Toiletries: shampoo, conditioner, hairbrush, toothbrush, toothpaste, deodorant, lotion Adults may bring: curling iron, electric razor, hair dryer Optional items: blanket/comforter, pillow, stuffed animal (in good repair) Bring enough items for 1 week - laundry services available on each unit. Lincoln Trail Behavioral Health System has a strict policy on restricted items to be brought into the hospital due to the safety of our patients. 2. What is the length of stay? The length of stay varies for each individual. The treatment team decides length of stay based on clinical information. All of our programs are short term except for the Willows program, which has a length of stay of 9 - 12 months. 3. How do I get to Lincoln Trail Behavioral Health System? Adults: Wednesday: 6:30 - 8:00 p.m. Sunday: 3:30 - 4:45 p.m. Adolescents: Tuesday: 7:30 - 8:30 p.m. Saturday: 1:15 - 3:00 p.m. 5. How do I contact someone if they are in the hospital? Every person that is admitted to our facility is given an ID#. This number must be used when attempting to call or visit someone at the hospital. Anyone trying to contact an adolescent must first be on an approved contact list from the guardian. 6. Do you accept adult Medicaid? No, the state of Kentucky does not approve freestanding psychiatric hospitals to accept adult Medicaid. A medical hospital with a psychiatric unit will be able to accept adult Medicaid. 7. Adolescent Chemical Dependency Program Admission Criteria The Adolescent Chemical Dependency Program serves patients who are 12 through 17 years of age. Treatment is deemed appropriate when any of the following criteria are met: The patient is deemed to be dangerous to self, others or to property. Reality testing is impaired and/or behavior is severely disordered. Alternatives to inpatient care have been unsuccessful or are not appropriate. Treatment involving pharmacological management and stabilization requires 24-hour skilled observation. The patient must reasonably be able to benefit from treatment by having a full scale IQ greater than 70. Adolescent Psychiatric Program Admission Criteria The following are criteria to be used in deciding whether or not to admit an adolescent patient for inpatient psychiatric care. These three conditions must be present 100% of the time: The patient is between 9 years and 17 years of age and is physically and mentally able to participate in the treatment program or has the potential to do so. With active treatment, stabilization of acute symptoms and/or improvement in the patient's level of functioning can be anticipated. The patient must reasonably be able to benefit from treatment by having a full scale IQ greater than 70. One or more of the following must be present 100% of the time: The patient poses a danger to self, others and/or property due to the behavioral manifestation of a psychiatric disorder. The patient needs continuous skilled observation and/or evaluation and/or treatment that can only be provided in a hospital setting. Due to a psychiatric disorder, the patient is impaired to the degree that he or she manifest substantial disability in social and/or familial functioning and this impairment cannot be treated in a lesser level of care. In addition to the above, the patient presents with one or more of the following: The patient lacks a social/family support system to such a degree that the indicated treatment is jeopardized. The failure of outpatient treatment and/or management has been verified. The inaccessibility of adequate outpatient treatment has been verified. The patient's condition can be expected to deteriorate with less intensive treatment. The patient manifests significantly impaired reality testing. The patient manifests significantly impaired judgment and/or logical thinking which is amenable to treatment. The patient manifests a significant affective disturbance. A medical/psychiatric evaluation is needed which requires a hospital setting. The patient has a concomitant medical condition or medication sensitivity, which complicates treatment of the mental disorder outside of a hospital. There is a possibility of aggravating or potentially harmful side effects of the patient's treatment of condition unless treated less that 24-hour observation. Psychiatric inpatient assessment is required because the patient has numerous psychiatric symptoms that conform to no definite diagnostic category. Adult Chemical Dependency Program Admission Criteria The following is admission criteria for the medically based inpatient chemical dependency program: The patient is assessed according to current DSM IV R diagnostic criteria. When the presenting history is inadequate to substantiate such a diagnosis, additional information submitted by the patient, family member or legal guardian could indicate a high level of probability of such a diagnosis. The patient has symptoms and/or behavioral manifestations of such severity that there is significant interference with social, family, vocational and/or educational functioning. The patient is found to be unmanageable in outpatient treatment alone and requires the protected observation and coordinated medical and therapeutic resources of inpatient programming. The patient presents with active suicidal or homicidal ideations. The patient must reasonably be able to benefit from treatment by having a full scale IQ greater than 70. In addition to the above, the patient presents with one or more of the following prior to admission: The patient lacks a social support system to such a degree that treatment is jeopardized. The failure of outpatient treatment and/or management has been verified. The patient's condition can be expected to deteriorate with less intensive treatment. The patient manifests significantly impaired reality testing. The patient manifests significantly impaired judgment and/or logical thinking which is amenable to treatment. A medical and/or psychiatric evaluation is planned which requires a hospital setting. The patient has concomitant medical conditions, which complicates treatment of the chemical dependency diagnosis outside of a hospital. The patient needs the 24-hour structured therapeutic environment provided in a hospital. Adult Psychiatric Program Admission Criteria One or more of the following must be present 100% of the time: The patient poses an actual or imminent danger to self, others and /or property due to the behavioral manifestations or a mental disorder. The patient needs continuous skilled observation and /or evaluation available only in a hospital. Due to a mental disorder, the patient is impaired to the degree that he/she manifests major disability in social and/or familial functioning, and this impairment cannot be managed in a less sensitive setting. The patient must reasonably be able to benefit from treatment by having a full scale IQ greater than 70. Absence of a severe medical condition requiring invasive medical techniques. In addition to the above, the patient presents with one or more of the following prior to admission: The patient lacks a social support system to such a degree that indicated treatment is jeopardized. The failure of outpatient treatment and/or management has been verified. The inaccessibility of indicated outpatient treatment has been verified. The patient's condition can be expected to deteriorate with less intensive treatment. The patient manifests significantly impaired reality testing. Career Opportunities Registered Nurse/Licensed Practical Nurse The nurse is a very significant part of the Multidisciplinary Team at Lincoln Trail Behavioral Health System. Some responsibilities of this position include coordinating nursing care in collaboration with other disciplines such as Psychiatrists, Addictionologists, Therapists and Case Coordinators. The nurse plays a vital role in communicating and interacting with the patients and their families. Other responsibilities may include administering medications, monitoring and reporting side effects, leading group sessions, maintaining a therapeutic setting and offering support and encouragement to patients at their most vulnerable time. Clinician/Therapist Another significant part of the Multidisciplinary Team, the clinician is responsible for a variety of direct clinical duties in the patient setting. Responsibilities include providing Individual, Group and Family Therapies, completing Psycho Social Evaluations upon admission, completing Master Treatment Plans and working closely with the Psychiatrists, Addictionologists, Nurses and Case Coordinators on implementing quality care. The clinician plays a vital role in allowing the patient and his/her family to work through a crisis, quickly stabilize and return to the appropriate level of care after discharge. Clinicians at Lincoln Trail Behavioral Health System must have a Master's degree and are preferably Licensed Clinical Social Workers. Mental Health Technician The Mental Health Technician (MHT) works closely with other disciplines on delegated assignments to meet the age specific physical, social and behavioral needs of the patients. Under the direction of licensed staff, the MHT ensures patient safety, fosters and actively assists in maintaining a therapeutic setting for the patients and is constantly interacting with the patients. The multidisciplinary treatment team seeks to engage the family as an ally in the adolescent's treatment whenever possible. Each adolescent attends school on Lincoln Trail's campus at Heartland Academy. Staffed by Kentucky Certified Teachers, the school offers a minimum of six hours of individual instruction and study daily. The adolescent's Case Coordinator begins discharge planning upon admission by developing goals and plans for a transition to a less intensive level of treatment. Adolescent Psychiatric Program The Adolescent Acute Inpatient Psychiatric Program at Lincoln Trail Behavioral Health System is provided to help adolescents ages 9 - 17, remain safe, reduce symptoms, and improve functioning. Specifically, the purpose is to: Reduce thoughts and behaviors that pose a danger to the adolescent or others Reduce severe symptoms associated with depression, conduct, psychosis and other psychiatric disorders Improve functioning in the adolescent's social settings such as home, school, church, etc. The program allows the adolescent and his/her family to work through a crisis, quickly stabilize and return to the appropriate level of care after discharge. Ultimately, the program helps the adolescent foster appropriate expression of emotion, self-control, anger management and development of positive peer relationships. Individuals are admitted to, involved in and discharged from the program based upon established clinical criteria. Comprehensive assessment and evaluation determine the adolescent's specific problems and diagnosis as well as strengths and resources. Collaborative treatment planning with the Psychiatrist, Case Coordinator, Social Worker and the Nurse helps each patient develop specific goals and determines interventions with the greatest likelihood of success. These interventions are symptom and solution focused. Interventions include the provision of a safe and therapeutic environment, daily physician supervision, nursing and monitoring, and care and intensive programming (i.e. Group Therapy, Psychiatric Education, Nursing Education, Intensive Family Therapy, Individual Therapy as needed and Activity Therapy). Additionally, a behavior management/ level system and linkage with an after-care professional are provided. The individual's Case Coordinator focuses on developing plans for a transition to a less intensive level of treatment.
